Dates in 2018 needed:
Orientation must be done on a WEEKDAY prior to first shift
Nov 4th 5:30a – Nov 12 6a
Nov 19 6a – Dec 10th 6a
Dec 21 5p – Jan 7th 6a
Procedures Required (types of procedures to be performed by locum tenens provider): Inpatient consults and rounding + Outpatient schedule M-F and call on Sat/Sun
Processing time for privilege application: 60-90 days for full privileges once file in 100% complete must go to committee meetings.

Provider must oblige to the following requirements:
Complete Locum forms (IMPACT, Pharmacy & Medical History Forms), provide a professional headshot, Complete on-line Training Modules (Carle-LMS) which takes approx. ~2 hours – locum needs to do prior to orientation.
Pre-employment Drug screening, Executive Physical, and Immunization Review on orientation day
Flu vaccine verification or needs to complete declination

Trauma Level: Level I Trauma Center; Level III Perinatal Center
EMR System: EPIC – provider MUST be efficient with EMR
