Practice Details
This is a long-term contract opportunity offering stability, competitive compensation, and the chance to work in a mission-driven federal healthcare system. Physicians will deliver comprehensive, patient-centered care aligned with American College of Physicians (ACP) and American Academy of Family Physicians (AAFP) guidelines.
You will be part of a 3-physician team, ensuring consistent on-site coverage and continuity of care within the Primary Care Clinic, while working in a collaborative, interdisciplinary environment.
Schedule & Work Expectations
- Monday–Friday, 8:00 AM – 4:30 PM (no nights)
- Approximately 40 hours per week per physician
- Start date: Anticipated assignment beginning July 2026
Patient Population & Care Setting
- Population: Adult and Geriatric Veterans
- Care Model: Patient Aligned Care Team (PACT)

- Outpatient Primary Care Clinic
- Preventive and Chronic Disease Management

Key Responsibilities
- Deliver comprehensive primary care in accordance with ACP and AAFP guidelines
- Diagnose and manage acute and chronic conditions (e.g., diabetes, hypertension, COPD)
- Perform preventive screenings and routine health maintenance
- Conduct mental health screenings and coordinate specialty referrals
- Lead care coordination within the PACT team model
- Educate patients on lifestyle modifications and disease prevention strategies
- Address geriatric care needs, including fall risk and cognitive concerns
- Provide care through both in-person and telehealth modalities
- Maintain timely and accurate documentation in the VA electronic health record (EHR)
- Review inpatient admissions and ensure proper clinical handoffs
- Adhere to all VA policies related to patient safety, infection control, and compliance
- Participate in staff meetings, peer reviews, and quality improvement initiatives
Required Qualifications
- Active, unrestricted U.S. Physician License (any state accepted)
- Board Certified or Board Eligible in Internal Medicine (ABIM/AOBIM)
- Eligibility for and successful completion of VA credentialing and privileging
- Strong clinical judgment, communication, and documentation skills
- Proficiency in written and spoken English
