Clinical Geriatric Practice
Mercy LIFE West Philadelphia, a member of Trinity Health- Mid Atlantic seeks a fulltime geriatric clinician for their PACE Program. The LIFE Program is an All-Inclusive Care for the Elderly (PACE) and is centered on the belief that seniors with chronic care needs and their families need to be served in the community.
- Provides primary medical care to PACE participants and works with all providers of the interdisciplinary team in a collaborative effort
- Position is 100% clinical and is located at 45th & Chestnut Street
- Provide support for the Associate Medical Director monitoring the status of all patients and utilization review programs
- Prefer candidates with geriatrics experience and those who can confidently lead and explain medical decision-making, while being a fair and helpful member of a larger group.
