PRACTICE SPECIFICS 
• Looking for a strong, patient-centered, team player with a desire to serve in a learning/teaching environment that will do broad-spectrum Family Medicine; OB and C-Sections are optional 
• Desire someone that loves the small-town atmosphere and a commitment to work/life balance 
• If this is you, join a current team of 8 Physicians, 3 APC’s and Additional Staff 
• Admit to adjacent Hayward Area Memorial Hospital: 25-bed, critical access hospital 
• Outpatient and Inpatient Practice 
• On site lab, pharmacy & physical therapy 
• Patient volume: 14-21 per day in the clinic 
• Current practice of 3 DO’s and 5 MD’s 
• Monday – Friday work week – 32 hours in clinic; 8 hours of admin and hospital work 
• Round at the local nursing home 
• Call is 1:11; schedule is approx. 3 days per month and 1 weekend a month (call is shared with other clinic in town) 
• Hospital has 24-hour ER coverage

• BC/BE Family Practice 
• OB optional, C-section optional 

• Location of Medical Training: American Medical Graduate (AMG), Foreign Medical Graduate (FMG)
• Medical Training Type: Allopathic (MD), Osteopathic (DO)
• Board Certification: Board Certified, Board Eligible
• Work Status Requirements: US Citizen, Green Card
