Period of performance is 1/1/16-12/31/16 Closing date to submit providers is November 22nd at 5pm One year option to extend 2017 Outpatient only M-F, 8a-6p 6 month minimum commitment # of patients per clinic: 5 – 8 new patients; 4 – 10 return patients # of procedures per surgical session: 5 – 8 cases per day BLS, ACLS, PALS DEA Scope of services includes: staffing of Outpatient Mohs Clinic; providing Mohs micrographic surgical procedures; follow-up care for Mohs surgical patients; and providing consultations as needed
