Horizon Health and Wellness  provides integrated behavioral health, primary care and wellness services throughout the southern half of Arizona.  We focus on the under-served including SMI, children and families and individuals in recovery.  Some of our locations are in smaller, more rural communities with others closer to the larger metropolitan areas of Phoenix and Tucson.  We are building a team of dedicated professionals who believe in the integrated healthcare model and are seeing the results of a focus on the whole person.  Our new clinic located in downtown Casa Grande offers both primary care and behavioral health services to the community.  

Provider will report directly to the Chief Medical Officer and work collaboratively with the other team members to develop medically sound treatment plans.  Must have an Arizona license.
