NEPHROLOGY
Our nephrologists share our passion for improving quality outcomes for health plan members suffering from kidney disease. They manage a much smaller panel of patients than in the typical nephrology practice, providing care in a way that promotes patient engagement long before ESRD, resulting in delayed disease progression.
For patients with kidney failure, our nephrologists prioritize informed patient choice, permanent vascular access, and planned dialysis starts or close medical management, depending on the clinically appropriate choice.
Our nephrologists work remotely from home and in person locally when necessary. As a part of working at a national practice, MDs will be licensed in multiple states. Physicians work on an employed basis, full-time, 5 days a week.
Responsibilities:
Collaborate with the Medical Director to
proactively identify CKD and ESRD members in predefined geographic region
Own all outcomes for patient panel driving both predetermined clinical and financial KPIs
Drive Clinical Interventions for Patient Panel:
Treat panel of patients both in-home and virtually as necessary
Apply evidence-based criteria in determining the necessity of care; whether dialytic or non-dialytic care is the correct pathway
Round on incident hemodialysis/ peritoneal dialysis members in clinic or in-home
Round on patient panel (if admitted) in local hospitals within a defined geographic market
Reduce unnecessary episodes of care
Identify and make practical pointed interventions to improve member care
Refer members to appropriate specialists
Conduct meaningful end-of-life treatment options with patients
Support Clinical Teams:
Work with local company physicians to drive KPI's
Participate in monthly sessions with the Regional Medical Director & CMO concerning expectations around KPIs
Share in backup coverage with other MDs
Maintain active privileges at local dialysis centers and hospitals
Review annual wellness visit assessments to ensure proper coding and provision of medical diagnosis across multiple comorbid conditions
Assist in the training of NPs and other field providers
Requirements:
2+ years of nephrology experience
Experience applying evidence-based clinical criteria
Experience in geriatrics
Comfort in advance care planning
Experience with high-need Medicare and managed Medicaid populations
Expertise in managing medication panels for complex patients
Current state medical license without restrictions
Willingness to become licensed in multiple states
Doctorate degree in medicine
Board-certified in nephrology
