Remote Medical Director opportunity in Phoenix
Excellent opportunity for an AZ licensed physician to be responsible for clinical oversight and administrative lead to help build and grow the market for one of the nation's leading risk-bearing healthcare organizations. This group provides on-site care and virtual visits to vulnerable seniors and adults in their homes and various settings throughout Metro Phoenix. You will be a part of a successful and innovative national team and help implement and improve care solutions to patients.
RESPONSIBILITIES
- Build/grow an innovative, high-performing primary care practice that carries an excellent reputation among communities of patients
- Recruit and hire the best clinical team, orienting them toward a value-based model of care that includes a strong focus on home-based care solutions
- Support market budgeting and forecasting
- Provide direct supervision, coaching and mentorship to a team of physicians and advanced care providers
- Direct delivery of home and community-based care (preventative, acute and chronic care management) on a consulting or continuing basis
- Ensure accurate and thorough clinical documentation for all patients
- Lead clinical education and quality improvement activities
- Spend at least 20% of time to care for the most complex patients both in person and virtually

REQUIREMENTS
- Active AZ medical license
- Board Certified in Internal Medicine, Family Practice or Pediatrics
- Additional board certification in Emergency Medicine, Gerontology or Palliative Care is preferred
- DEA certification
- 3 years post-residency clinical experience delivering care to complex patients including in the home and skilled nursing facility settings
- Demonstrated ability to collaborate both internally and externally with multi-disciplinary care team members
