Practice Details
Large post acute practice one hour west of Albany NY is looking to add a primary care physician. You would work Monday-Friday 8am-5pm and will take call all 1 weekend per month. Post-acute care is a special level of care not offered at acute-care hospitals. When patients require post-acute care they are transferred to a facility where the care is tailored to their individual goals. A team of health care professionals including rehabilitation specialists will work with the patient and his or her family to make sure their goals for recovery are achieved.
