• Clinic of 16 providers including family medicine physicians and nurse practitioners partnering to deliver team based care.
• Certified Medical Home with onsite chronic disease management nursing support and care coordination specialists
• EPIC electronic medical record
• PACS radiology and a high complexity in-house laboratory help provide state-of-the-art care

• Board Certified/Board Eligible Family Medicine with OB
