REQUIREMENTS:

1. Current and unrestricted license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia.
2. Currently Board Certified/Eligible in Family Medicine or Internal Medicine.
3. Current Basic Life Support (BLS) certification.
4. Current Drug Enforcement Agency (DEA) registration.
