We provide outpatient medical, dental and integrated behavioral health services. We have an experienced leadership team and proudly received national recognition as a Level 3 Patient-Centered Medical Home by the NCQA. Our organizational goals include attaining high patient health outcomes and staff and patient satisfaction. Our staff are committed to providing high quality healthcare with a heart. We are looking for a general dentist to provide care across the lifespan on a part-time or long-term locums basis.
