* % of practice: 95% Adult, 5% Trauma

* Ave. no. of clinic patients per day: 13 - 16; Scheduled - 14; Add-ons - 1 to 2

* Ave. no. of surgeries per day: 4 - 6

* No. of physicians in group: 2; Mid-levels - 3

* Provider is required to supervise PAs

* Lab and X-ray on-site

* Support Staff: 1 Clinic RN, 2 Clinic MAs

* Hospital privileges required: Yes

* Time needed to obtain privileges: 30 days

* Will consider BCGS or Cardiothoracic with Vascular experience
